A benefit for Medi-Cal members to get you the medical care you need.
Through Enhanced Care Management (ECM), eligible members are assigned their own care team from one of our qualified providers.
This team includes a lead care manager who will talk with doctors, mental health providers, specialists, pharmacists, social services providers, and others. The lead care manager will make sure everyone works together to get you the care you need. They can also help you find and apply for other services in the community.
ECM services include:
Finding medical providers
Scheduling appointments for health-related services
Managing medications
Setting up transportation to the doctor or pharmacy
Assistance finding and applying for community-based services, including housing and food
Setting up follow-up care after a hospital visit

Community Supports (CS) are other services for Medi-Cal members that may not include direct medical care. Central California Alliance for Health offers Community Supports to help members with complex health issues that are often caused or made worse by a lack of food, housing or transportation. CS services include:
CS offers housing services that include:
Transition Navigation Services
- Assistance with securing housing, including the completion of housing applications, and securing required documentation (e.g. Social Security card, birth certificate, prior rental history).
- Additionally CS workers can communicate and advocate on behalf of the Member with landlords. All together this service ensures that the living environment is safe and ready for move-in.
Deposits
- Aiding clients with security deposits required to obtain a lease on an apartment or home, including set-up fees and deposits for utilities/services access and arrearages.
- This is a $5,000 maximum lifetime benefit for clients.
Tenancy + Sustaining Services
- Provision for tenancy and sustaining services with the goal of maintaining safe and stable tenancy for the client once housing is secured.
- We provide early identification and intervention for behaviors that may jeopardize housing, such as late rental payment, hoarding, substance use, and other lease violations.
- Our team also works on independent living and life skills with clients. This includes assistance with and training on budgeting, such as financial literacy and connection to community resources.
To qualify, you must be:
An individual or family experiencing homelessness or unstable housing
An adult who has complex health issues and has needed to go to the hospital or emergency room many times over the last 6 months
An adult who has a serious mental health condition or has a hard time with drug and alcohol use
